3: Date of Birth for a Veteran is required for all Veteran profiles. Please ensure you have a correct Date of Birth before adding a new veteran.
4: Only veterans themselves, their next of kin, or their authorized representatives should submit information through this web form. The Veterans office will verify all entries. We seek to honor veterans who wish for their profiles to be included. We do not want to disclose any information a veteran does not want to be made public.
